Responsibilities

Essential Functions

 

  • Organizing and managing the multi-facility MDS process for all buildings. This includes:
    • Conducting all job responsibilities in accordance with the standards set forth in TLC Management’s Code of Business Conduct, applicable federal/state laws and professional standards;
    • Being available and responsive to the CRAC, RDO and RDQA, maintaining constant communication with them;
    • Being available and responsive to each facility’s MDS Coordinator;
    • Sitting in as the acting MDS Coordinator when an MDS vacancy occurs in any of the assigned facilities;
    • Assisting with the interviewing and selection of new MDS Coordinators;
    • Monitoring and tracking facility CMI scores and HP scores for improvement recommendations;
    • Conducting MDS in-services, corporate conferences and other needed in-services regarding regulation and systematic changes;
    • Implementing and ensuring systematic usage of MDS policies/procedures throughout the region;
    • Working and collaborating with other RRACs/CRAC to ensure consistency in practice and success of all MDS protocols for TLC Management;
    • Orientating and training of all MDS Coordinators concerning accurate documentation and regulatory changes. This includes making recommendations for needed in servicing to DON and RDQA;
    • Ensuring that MDS Coordinators are completing all annual, quarterly and significant changes in MDSs;
    • Ensuring that each MDS Coordinator is updating care plans and coordinating care plan reviews upon admission, annually, quarterly and with significant change;
    • Ensuring completion of the Care Area Assessments required according to MDS assessment findings;
    • Reviewing care plans for appropriate goals, problems, approaches and revisions based on nursing needs;
    • Ensuring that each facility is preparing and implementing appropriate procedures to meet the needs of the HP audit and other accuracy and financial audits;
    • Monitoring compliance with daily PPS meetings in assigned buildings;
    • Monitoring compliance with weekly Medicare meetings in assigned buildings;
    • Monitoring assessment scheduling according to Medicare Guidelines and ensuring compliance with Medicare billing through the Triple Check process is occurring in assigned buildings;
    • Attending scheduled in-services, staff meetings, workshops and seminars as required and assigned. This also includes attending seminars and training specific to furthering MDS education as needed;
    • Assisting to identify and correct BC2 days on case mix reports in a timely manner;
    • Assisting to identify and correct default days on monthly Medicare bills in a timely manner;
    • Maintaining knowledge of policy and procedures related to care plans, MDS and Reimbursement protocols;
    • Being prepared to handle emergencies as they come up (i.e., rescheduling work assignments and work schedules, etc.).
    • Maintaining the confidentiality of all residents and their care to ensure resident rights and privacy are protected at all times; and
    • Reporting and documenting resident and/or visitor incidents, accidents, complaints or allegations according to the facility’s policy and procedure using the designated occurrence reporting system;
  • Complying with safety requirements, infection control measures, and maintaining a clean work environment in accordance with facility policy as indicated by:
    • Abiding by all emergency protocols for fire safety, missing residents, tornado preparation, violent residents, Wander guard alarms, door alarms, and other facility policies as assigned;
    • Reporting any forms of abuse witnessed or aware of to the nurse in charge immediately; &
    • Following infection control and universal precautions procedures.
  • Performing other duties as assigned by the Corporate Resident Assessment Coordinator, Regional Director of Operations, RDQA or VP of Operations. This may include sitting in as interim DCS if RAC nurse is unavailable.

 

Qualifications

Qualifications

 

  • Must possess specific educational and experience requirements such as:
    • A graduate of an accredited school of nursing and possess a valid RN or LPN license in good standing according to State and Federal requirements.
    • Must possess at least two (2) years of previous MDS Coordinator experience.
  • Must understand the philosophy of and know the principles of the MDS process.
  • Possession of a thorough understanding of the principles of safe, effective nursing practices.
  • Must possess managerial and interpersonal skills such as:
    • Have knowledge and understanding of computers and software (i.e.; Word, Excel, & etc.);
    • The ability to follow written and verbal instructions, function independently and make independent decisions when circumstances warrant such action;
    • Personal integrity and the ability to deal tactfully with residents, family members, visitors, government agencies and personnel;
    • The ability to work harmoniously with professional and non-professional personnel;
    • Requires judgment in prioritizing the performance of tasks, duties and responsibilities in accordance with established standards, policies and procedures;
    • The ability to be discrete and protect the integrity of confidential information and stay within Corporate Compliance and HIPPA;
    • Patience, a cheerful disposition, enthusiasm and a willingness to handle residents based on whatever maturity and physical level in which they are currently functioning;
    • The ability to relate information concerning a resident’s condition to the appropriate staff members.
    • The ability to adhere to professional code of ethics, and

    • The ability to work Monday-Friday during normal business hours and the ability to take call and come in during off shifts as needed (i.e. weekends, midnights, and holidays); and
    • The ability to travel consistent with demands of the position and capable of performing the essential job functions of this job with or without reasonable accommodations. This also includes the ability to travel throughout the state under diverse weather conditions.
