As an MDS Navigator with TLC, you’ll be at the heart of our continuum of care—partnering with interdisciplinary teams to ensure accurate, compliant assessments that drive optimal resident outcomes. In this role, you will guide clinical documentation, enhance reimbursement accuracy, and champion care plans that support our residents’ journey to living well—because at TLC, we believe “when you live well, you are well, and that changes everything.” 

Responsibilities

Key Responsibilities

  • Organize, manage, and complete the full MDS process for all residents in compliance with state and federal guidelines.
  • Attend daily morning meetings to monitor changes in orders, payors, and resident status; lead or assist in care plan meetings as directed.
  • Conduct daily PPS meetings and weekly IDT rehab meetings to manage Medicare, Managed Care, and Medicaid processes.
  • Maintain and communicate MDS schedules; complete all required PPS, OBRA, annual, quarterly, significant change, and discharge assessments.
  • Accurately code and sequence diagnoses; formulate and update care plans based on MDS findings.
  • Ensure timely transmission of MDS data, reconcile errors, and audit documentation for accuracy.
  • Monitor QM reports for coding issues and resident concerns; prepare for audits and maintain compliance with Myers and Stauffer requirements.
  • Provide emotional support to residents and families, handle concerns professionally, and maintain confidentiality.
  • Comply with all safety, infection control, and emergency protocols; attend required in-services and training.
  • Perform additional duties as assigned by the Health Facility Administrator or corporate leadership.

Qualifications

  • Education & Licensure:
    • Graduate of an accredited school of nursing with a valid RN license in good standing (per State and Federal requirements working).
  • Experience:
    • Minimum of 2 years recent nursing experience.
    • Strong understanding of the MDS process and its guiding principles.
    • Thorough knowledge of safe, effective nursing practices.
  • Technical & Professional Skills:
    • Proficient in computer applications such as Microsoft Word and Excel.
    • Ability to follow written and verbal instructions, work independently, and make sound decisions when necessary.
  • Interpersonal & Leadership Skills:
    • Demonstrates personal integrity and professionalism.
    • Skilled in tactful communication with residents, families, visitors, and regulatory agencies.
